Our goal is to provide you with the best service and make your entire experience with us as comfortable and informative as possible.

Patient Forms

To facilitate your initial visit, we ask that you fill out the Patient Information and Medical History Form prior to your appointment. You may download, print and complete this form, and then bring it with you on the day of your consultation. We can also mail it to you, along with our office brochure and any pertinent educational material, after your appointment has been scheduled.

You will be asked to read and sign several other forms upon arrival at our office, many of which are required by Federal and State government regulations.

Patient Portal

You can communicate with our office through the message center of the portal. Once you create your portal account, you can email us for many non-urgent needs. You may also receive messages or news from our practice, review and update your information, see when your next appointment is scheduled or view your medical record. A note will be sent to your email address alerting you to a message on the portal.

Financial Policy

Dr. Schwartz’ initial consultation fee is $100. When surgery is scheduled within six months of the consultation, this fee will be applied to the cost of surgery. Full payment for cosmetic surgery is expected two weeks prior to the surgery date. For your convenience, certified checks and all major credit cards are accepted.

Reconstructive surgery patients, whose procedure is covered by insurance, will be informed of their financial responsibilities prior to the date of surgery.
